E-Shop Item Return Form
We hope that returns will be minimal due to the entirely online ordering system, however, we understand that sometimes returns are necessary. Personalized items with your department, name or other artwork cannot be returned unless there is a manufacturing error. We simply cannot re-sell personalized items. If approved for return, the item must not be worn and still have original tags attached. Returns will be done on a case-by-case basis. Items must be returned within 60 days.
Easy to do:
1. Print and complete THIS FORM
2. Return to Bay Pride E-shop, Marketing & Public Relations Department, 503 Mulholland, Bay City. Put the completed form and item in the “Check Out Box” in the E-shop room.
3. Allow 7 days for processing of return request.
4. Email notification will include a return credit code for reordering or an explanation that the return was not accepted and instructions for picking up item.
Completion of this form does not guarantee a return/credit.
